The annual wellness visit is intended to focus on the prevention of disease and your overall health and well-being. Our team will provide all patients with a yearly in-person wellness visit. Most insurance plans cover this visit without a co-pay.
  • Routine Physical Examination
    • Update your medical, family, and social history.
    • Review and reconcile your medications and allergies.
    • Provide appropriate and important health screenings.
    • Review and update immunization records.
    • Discuss healthy eating, physical activity, stress management, and sleep pattern.
  • Initial Preventative Physical Exam (IPPE)
    • Review of medical, social health history and preventive services education.
    • New Medicare patients within 12 months of first the Part B coverage period.
    • Patients pay nothing (if provider accepts assignment).
  • Annual Wellness Visit (AWV)
    • Visit to develop or update a Personalized Prevention Plan.
    • Perform a Health Risk Assessment.
    • Covered once every 12 months.
    • Patients pay nothing.
  • Same day Sick Visit
    • Pre-exposure Prophylaxis (PrEP)
    • Flu/Cold Pneumonia
    • Urinary Tract Infections (UTI)
    • Sexually Transmitted Infections
    • Strep throat (rapid test available)
    • Pregnancy Tests
  • Walk-In
    Walk-ins are always welcome, but for your convenience, you can also register online and reserve a time slot that fits your schedule. We provide treatment for adults and children 6 years and older. Treatments include:
    • Colds and Flu
    • Earaches and Sinus Infections
    • Animal Bites and Insect Bites
    • Stomach upset and Diarrhea
    • Urinary Tract Infections
    • Sports Physicals
